As I understand it, <caption> should immediately follow <table>.
This was why I thought it should be at the end of the

||attributes !with a caption

rather than force pmwiki to deal with a possible error:

||!a header||and text||
||!!and a caption in the wrong place||

it's better to prevent the error in the first place

To handle alignment options, it may be better to require a closing !

||!This is the default (centred)!

||! This is right aligned!

||!This is left aligned !

||attributes
||!caption!

would be OK by me

Which is a good reason for dropping the ! in <th> markup:
